Print the following and read all the information carefully: 2. Collect the DNA samples, complete and sign the Consent Form. PLEASE NOTE: In order to collect DNA samples, you will need 2 sterile cotton-tipped swabs for each subject (can be purchased at most pharmacies or drug stores). This option allows you to save time and money. Alternatively, we can ship you the swabs. E-mail us to request the swabs for DNA collection, and we will send you a Paypal electronic invoice. IMPORTANT NOTE: Before sending the envelope with your DNA samples, we strongly advise you to contact us. We can review the whole DNA collection process, clarify additional doubts, or discuss other issues about your unique portrait art in greater detail. There are aspects of the DNA portrait that can be further customized as the order of the DNA samples on the portrait. Also, the time-frame for receiving the DNA samples is important. 3. Send the completed Consent Form and DNA samples to: Geneportrait 4. Once the envelope with the DNA samples arrives, we will send you a confirmation e-mail asking you to make the payment to start the process of creating you unique DNA-art portrait. 5. PAYMENT: Select the number of subjects that will be on the DNA portrait and click the "Add to Cart" button. You will then be taken to PayPal's secure ordering service. We never see, receive or handle your credit card or bank information. For a DNA portrait of more than 10 subjects, contact us and we will send you a Paypal electronic invoice.
6. Confirmation e-mails will be sent after receiving DNA samples + Consent Form, payment and once the DNA portrait CD has been sent out. We want to make sure that an honest and fluent communication channel is established between Gene-Portrait and its clients. We will always be there for you. |
